Preemies and babies born with medical conditions generally stay in the neonatal intensive care unit (NICU) after birth and are considered high risk for medical and developmental challenges that will require ongoing monitoring, assessment and multispecialty treatment.

DMG CRS has been caring for more than 16,000 babies and children of all ages with specialized medical needs annually since 2012. Who is part of the HRIF Clinic?

  • Social worker: Provides family support, care coordination, community resource referrals, advocacy, and emotional support

How is this visit different from seeing our regular pediatrician?

This visit is a comprehensive evaluation focusing on your child’s past medical history, developmental capabilities, and behaviors. We look for conditions that we know are common in children who have spent time in the NICU or who are considered high risk for neurodevelopmental diagnoses. Our developmental testing goes beyond the usual screening performed during a general pediatric visit. You may be asked to complete a developmental and/or behavioral questionnaire.

What are my next steps after discharge from the hospital?

  1. If the hospital referred you to the DMG CRS HRIF Clinic upon discharge, you will receive a phone call from our specialty care coordinator within 1-2 weeks to schedule your infant’s initial evaluation.
  2. If the hospital did not refer you to the DMG CRS HRIF Clinic, you may request a referral from your pediatrician or primary care provider (PCP) for an evaluation of your baby. The referral must be accompanied by the most recent office visit notes; referral phone and fax numbers are listed below. We will work with your child’s PCP to coordinate care and promote the growth and development of your baby.

What will my baby’s evaluation entail?

How often will my child need to follow up?

  • Clinic visits are usually made when your baby is referred, and at the ages of 3, 6, 9, 12, 18, and 24 months

  • Your child’s care team will structure a personalized plan of care based on the initial evaluation

  • Additional appointments may be scheduled as needed or requested by your child’s PCP
